A required consent and medical form will be made available by home schools prior to students’ Outdoor Lab trips. The form asks for basic medical information that will allow nurses to prepare for individual student needs.
Parents can contact their home school’s nurse if their student has specific health needs they are concerned about. All district nurses will work as a team with the Outdoor Lab nurses and staff to create a plan.
This Outdoor Lab Medication Agreement Form is required for any student bringing medications, vitamins, inhalers or epi-pens on site. The form requires a physician’s and parent or guardian's signature and is due to the student’s home school health room two weeks prior to departure.
The below forms, if needed, should be completed and turned in at the home-school health room.
